CONNECTICUT CARE · COVERAGE QUESTIONS
Find the right coverage question
Use the exact product name and the reason for treatment when asking about payment.
Begin with the product and the purpose
| Payment route | First question | Then check |
|---|---|---|
| Your insurance plan | Is this exact product covered for the reason it is being prescribed? | The plan’s medicine list and any approval step. |
| Cash payment | What is the full cost without a claim to my insurer? | The later price, other fees and what does not count toward insurance limits. |
| Medicare | Does this belong under the usual medicine coverage or the separate temporary program? | The treatment purpose, eligibility and required product form. |
Words to use with an insurer
A formulary is the plan’s list of covered medicines. Prior authorization is the plan’s review before agreeing to payment.
Ask about both using the exact product name and why it is being prescribed. Ask what records the plan needs and what happens after a denial.
Medicare has a separate temporary route
Part D is the name of Medicare’s usual medicine coverage.
Under the September 2026 rules, ordinary Medicare Part D excludes medicines used to lose weight.
As of September 2026, ordinary Part D may cover the same medicine for another accepted purpose, such as treating diabetes, but the plan still has to approve payment.
Your body makes GLP-1, a hormone that carries a chemical message to help control blood sugar and appetite. Medicines that copy its action are called GLP-1 medicines.
$50 is the monthly medicine payment for people who qualify for the temporary Medicare GLP-1 Bridge program, as of September 2026.
The program has a July 1, 2026 start date and a December 31, 2027 end date. It uses a payment system separate from ordinary Part D.
As of September 2026, Part D’s yearly cap on your payments excludes the Bridge’s monthly payment. Reaching that limit does not remove the Bridge’s $50 monthly charge.
The September 2026 Bridge rules require a prescription, approval before payment, and ongoing changes to eating and physical activity as part of treatment.
Check whether you meet the program’s rules
Under the September 2026 rules, you must be 18 or older. You need medicine coverage through either a separate Part D plan or a coordinated-care Medicare Advantage plan.
A coordinated-care plan manages your health care through a group of care services. Ask your plan to confirm its type before relying on the program.
The September 2026 rules bar Bridge eligibility for 2026 if Part D pays for your GLP-1 medicine at any point during 2026.
BMI is a number calculated from height and weight.
Ask the person treating you to check your BMI and health conditions from when you began GLP-1 treatment.
The following weight-and-health rules were checked in September 2026.
- For the weight-and-health requirement, a BMI at or above 35 is enough; none of the extra conditions below is needed.
- With a BMI of 30 or higher, you can qualify with heart failure of the type called “preserved ejection fraction”. The heart still works poorly even though the share of blood it pumps out is normal.
- With a BMI of 30 or higher, you can qualify with high blood pressure that stays above the program’s limit despite two medicines. The program uses these blood-pressure limits: a top number above 140 or a bottom number above 90. Kidney disease recorded as stage 3a or worse also qualifies at this BMI.
- With a BMI of 27 or higher, you can qualify with pre-diabetes. At that BMI, a past heart attack, stroke, or narrowed arteries in the arms or legs causing symptoms also qualifies. Blood sugar in pre-diabetes is above normal but below the level for diabetes.
Kidney disease stages describe how much kidney trouble you have. Ask your doctor to confirm the stage or the type of heart failure in your medical record.
When the ordinary Medicare plan is the route
As of September 2026, type 2 diabetes rules out the $50 Bridge option. Ordinary Part D may cover the medicine when it is prescribed for diabetes, even if weight is also a concern.
Under the September 2026 rules, moderate or severe obstructive sleep apnea also rules out the Bridge. This condition blocks breathing during sleep. MASH is another exclusion: liver inflammation from fat alongside moderate or advanced scarring of the liver, without cirrhosis. Cirrhosis is the most severe form of liver scarring.
A prescription to reduce the risk of serious heart and blood vessel events goes to the ordinary Part D plan under the September 2026 rules.
As of September 2026, ordinary Part D may cover a medicine for an accepted purpose other than weight loss. Payment still depends on the plan’s medicine list and approval; it is not guaranteed.
Check with your prescription writer and your plan about the route that fits your diagnosis. Ask which product and form the route allows; the official program pages give those details.
